Our Current Service Provision
Enabling Care For You Ltd currently delivers a wide range of community-based care services across Kent.
We have extensive experience supporting a wide range of discharge pathways, working closely with health and social care partners to ensure safe, timely transitions from hospital to home. Our service has consistently delivered responsive, high-quality care across Pathways 1, 2 and 3, adapting quickly to changing demand and complex patient needs. Most notably, we have recently worked in partnership with Maidstone and Tunbridge Wells NHS Trust to support more than 700 same-day discharges, helping to reduce hospital pressures while maintaining safe and effective care in the community. Our ability to mobilise care packages rapidly, communicate effectively with multidisciplinary teams, and maintain continuity for patients has been central to this success. We accept referrals 7 days a week and don’t close unlike our competitors.

To strengthen community care provision in coastal and island areas where sourcing care can be particularly challenging.

Bridging and Interim Support
One of the key strengths of our service is our ability to respond quickly and
provide immediate support. We regularly assist where:
- Care packages are not yet in place
- Existing arrangements have broken down
- Urgent or short-notice care is required
This ensures individuals remain safely supported at home, reducing the risk of escalation and maintaining stability.
