NHS Continuing Care

This is a service is when there is an individual with a long-term health condition has been assessed as eligible for ongoing care at home, we can provide all the homecare services required to enable that person to stay at home.

We can assist with

Personal care;
Prompts or administration of medication;
Cleaning, shopping and cooking;
Budgetary matters, letters and correspondence;
Companionship and befriending;
Accessing community resources and leisure activities;
Transport to appointments and meetings

Enabling Care For You believes that, wherever possible, people should be enabled to live in their own home with family or friends able to support them. We are committed to providing flexible and special services to enable this to happen.

Enabling Care For You provides respite services to enable carers to have time to themselves. This could involve overnight stays or longer periods to cover holidays or sickness.

Enabling Care For You specialises in supporting individuals with mental health needs, physical disabilities, sensory impairment, diabetes, epilepsy, dementia and end-of-life care.

All services will be based on the following objectives:

The promotion of personal choice and independence;
The development of social skills to realise maximum potential;
The commitment to person-centred planning and delivery;
The need to be responsive and flexible at all times;
To ensure the maintenance of a risk-free, safe and stimulating environment;
To empower service users through participation;
To work in partnership with all other individuals and agencies involved in the individual’s life;
To promote self-confidence and enablement that will ensure the individual’s potential is realised and the individual can continue to enjoy the highest quality of life possible.
Funding

Enabling Care For You can provide services to any individual who is funded by a managed account, direct payments or privately. Under direct payments, you will have the flexibility to purchase the service you desire and make decisions about how your care is delivered and choose who delivers it.

If you are already receiving support paid for by Social Services, you can ask to get direct payments instead.
