Bridging Care

This service is designed to support the individual while they are waiting for a permanent placement in a care home or just requires more support to stay in their own home. A holistic approach also enables us to provide support that looks at the whole person as an individual, not just their mental health needs. The support also considers their physical, emotional, social and spiritual well-being.

Since September 2024, we have been proud to support patients returning home safely following hospital admission, working closely with both Maidstone Hospital and Tunbridge Wells Hospital discharge arrangements.

Our focus throughout has been to provide a responsive, reliable service that enables timely discharge while ensuring each individual receives the appropriate level of care and support at home.

Consistent Delivery and Capacity

Between September 2024 and mid-2025, we delivered 43 consecutive weeks of uninterrupted service. During this time, we supported 1,164 individual clients, achieving an average of 27 discharges per week.

The care provided ranged from single-handed visits to complex, 4 x daily (QDS), double-handed packages. This reflects both the scale of demand and our ability to deliver flexible, person-centred care across a wide range of needs.

Responding to Increased Demand

From 27th October 2025 onwards, we have continued to support a further 788 patient discharges, with activity increasing to an average of approximately 36 discharges per week.

These figures do not include attempted discharges that did not proceed, indicating a consistently high level of operational demand and involvement in supporting patient flow.

A Responsive and Collaborative Approach

Throughout this period, we have maintained a flexible and responsive approach, working across various funding streams to ensure continuity of care and timely discharge support.

We have worked closely with hospital teams and wider health and social care professionals, adapting to changing demand and requirements while maintaining a consistent standard of care. This collaborative approach has been key in supporting safe discharge planning and reducing delays.

The success of this service is built on the dedication and professionalism of our care team, who have consistently delivered high-quality care in often fast-paced and changing environments. Their ability to respond quickly, manage complex packages, and maintain a compassionate, person-centred approach has been central to achieving positive outcomes.

We also recognise the significant efforts of hospital flow and discharge teams, whose coordination and planning are essential in ensuring patients can leave hospital safely and efficiently. Strong communication and partnership working between all teams have been fundamental to the effectiveness of the service.

An important measure of success has been the number of individuals who have chosen to continue their care with us following their initial discharge support.

Many clients and families have expressed confidence in the quality and consistency of the care provided, leading to ongoing care arrangements beyond the initial discharge period. This continuity supports improved outcomes, offering reassurance, familiarity, and stability during recovery at home.
